Please find attached a Club Member Information Form which needs to be filled in by all new members and any existing members who wish to provide changes to their information as this will also be used to produce the Who’s Who which will published early in the season. Forms can be scanned and emailed to firstname.lastname@example.org or left at the bar for the Registrar to collect. All members will also be required to have a photo taken to be included in the Who’s Who.
Member fees for 2015 are $140.00 for a Playing member and $50.00 for a Non-playing member. New members will also be required to purchase a Club shirt and shorts from the Club and buy their own plain Navy Blue socks available from retail outlets.
Fees can be paid in cash at the bar or by EFT into the Club account BSB 086495 Account Number 284310464 and the payment should be referenced with at least the member’s surname.
New players need to also complete the attached AFL Masters Registration Form and agree to the AFL Masters Philosophy and Player Consent conditions on the AFL Masters WA website at http://www.wamastersfooty.com.au to be registered BEFORE they are eligible to play.
Players who have been previously registered can login with their registration number (in the format M07/999) and a password to keep information up to date. The default password should be changed to a personalised one to protect the information on the AFL Masters WA website. AFL Masters WA rely on information being correct, especially the contact information including the email address, in order to communicate on important matters and for players to register for the National Carnival held each year. If a password is forgotten then the Forgotten Your Password facility can be used to get the password reset which should then be changed to a personalised one.
